During your first visit, you will be asked to complete forms asking about your medical history and history of your orthopaedic injury. You will also complete several questionnaires about how your injury affects you. Please arrive at least 30 minutes prior to your scheduled appointment to complete these forms. Alternatively, if you have MyHSS, you may have the opportunity to complete the questionnaires online prior to your visit. You can sign up for MyChart here
When you come for your appointment, please bring the following
- Driver’s License or a valid ID
- Insurance information
- Referral Letter (if required)
- Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
- List of medications (if any)
